Please follow the instructions below:

1. Please click here to download the application form as a pdf.
2. To print the form correctly:
   1. Go to “Print Setup” and ensure that paper size is “A4” (not “Letter”!)
      
   2. Do not use the print icon!
      
   3. Go to “File” then choose “Print…” (or press Ctrl+P).
      
   4. Set Page Scaling to “None”. Clear fields “Auto-Rotate and Center” and “Choose Paper Source by PDF page size”
